CLEVELAND , Ohio - In the weeks leading up to the April 28 murder of David Perez , 19 Investigates found police had numerous interactions with his suspected killer and had even tried to take away his guns.

Seventeen days later, police said Rich hopped into his car, followed David Perez and his wife Jackie onto Interstate 77, and“I’ll never know why it happened,” Jackie said in an exclusive interview with 19 Investigates. “He doesn’t know how he destroyed our whole life and I’ll never know why.

On June 13, 2022, a Newburgh Heights police officer filled out an emergency application have Rich committed for psychiatric care at St. Vincent Charity Medical Center.“NHPD has had several dealings with Ian Rich. Ian has presented both auditory and visual hallucinations. Ian states that his elderly neighbors are trying to murder him.

On Feb. 27, a mobile crisis team showed up at Rich’s house after receiving a call from his mother who was concerned about his behavior. The social workers asked police to respond as well because they were told he had guns and a history of using meth.“They were told that he believed that the FBI was watching him and that ghosts were letting his dog out of his house. They also stated that he has guns in the house and that he has been destroying the inside of the house and dressing inappropriately.

About two weeks later, officers returned with an order from the Cuyahoga County Probate Court to take Rich to a hospital for a psychological exam.“I can’t have another psych ward thing, where they put me in the psych ward. I can’t have that ever again,” Rich said angrily. Officer Halaska then escorted Rich to an ambulance. She again tried to get him to turn his guns over to police but he refused.

According to Rich’s mother, he went on a rant about his neighbors and said needed to kill them. When Rich’s brother told him to stop, she said he physically assaulted him. “And then they left, and it was a matter of minutes before David wasn’t here anymore because he followed them right out of the yard,” Sally said.Police said Rich was behind the wheel of the Audi convertible that can be seen on video following them onto the highway.

“If all this was happening, why couldn’t anybody do anything? I don’t get that. I don’t understand that,” Sally continued. “And then knowing he had guns in the house, how could you know that someone has a mental problem, and he was on drugs, they knew he was on drugs! How could you allow somebody like this to have guns in the house?”
