A GP has admitted making "a mistake" in referring a man for a mental health assessment just months before he attacked and killed his granny, an inquest has heard.

I should have been much more clear."
"I established that Conor had assaulted Mr Kennedy by hitting him with a can of lager," PC White said. "It was unprovoked and there was no explanation for it."
