Researchers compared metastasis-directed therapy plus systemic therapy with systemic therapy alone in patients with pancreatic cancer.

Adding metastasis-directed therapy to systemic therapy improves progression-free survival to 10.3 months vs 2.5 months with systemic therapy alone. The study involved 41 patients with oligometastatic pancreatic ductal adenocarcinoma .Researchers conducted a phase 2 multicenter, randomized basket trial involving 41 patients with oligometastatic PDAC in the EXTEND trial.

MDT included definitive local therapy, with stereotactic ablative radiotherapy recommended when feasible. Members of the control group were allowed to receive MDT if their disease progressed. "Secondary endpoints included overall survival, time to next-line systemic therapy, time to local failure, time to new lesion formation, toxicity, and quality of life," the authors said.According to the authors, adding MDT to systemic therapy significantly improved PFS to 10.3 months vs 2.5 months with systemic therapy alone , indicating a substantial reduction in the risk for disease progression or death with MDT.
