Mark S. Lesney, PhDsurvivors discharged to post-acute care facilities are at high risk for mortality and hospital readmission, according to Nicholas Colucciello, MD, and few interventions have been shown to reduce these adverse outcomes.
"Over one third of sepsis survivors are discharged to post-acute care as they are not stable enough to go home," said Colucciello, and among these patients there is a high risk for mortality and hospital readmission.
