The IG investigation involved interviews with 33 witnesses, providing insights into the circumstances surrounding the incidents that prompted the investigation. On April 3, 2024, while returning from Ukraine to Germany, Aguto was transporting classified maps. These maps, due to their size, were stored in a cylindrical tube rather than being secured with the required double-wrapping or a locked container, a clear violation of established security procedures. The travel party departed the train upon arriving in Poland on April 4, leaving the tube containing the classified documents behind. Although not the designated person to manage the materials, Aguto took responsibility for the temporary loss of the maps, which were ultimately recovered by a Ukrainian train attendant and delivered to the U.S. Embassy within 24 hours. The incident underscores lapses in adherence to crucial protocols designed to protect sensitive information, potentially jeopardizing national security interests.\The second area of concern detailed in the report centers around a series of alcohol-related incidents during a nine-day trip to Ukraine on May 13, 2024. During a military event, Aguto consumed two 500 ml bottles of Chacha, a Georgian brandy known for its high alcohol content (40-50%). Later that evening, he suffered a concussion after falling backward in his hotel room. The following morning, he was noticeably incoherent during a high-level meeting attended by then-Secretary of State Antony Blinken, the U.S. Ambassador to Ukraine, and Ukrainian military generals. Witnesses reported that Aguto appeared disheveled, confused, and acted in a manner described as 'not himself.' His behavior raised concerns, with the U.S. ambassador even suggesting he may have been drugged. The IG report detailed that his appearance was disheveled, his eyes were bloodshot, he had a large and visible wound on his elbow, he spoke more slowly, he seemed confused, he asked the same question repeatedly, and he frequently returned to the same topic. The report concluded that the falls and his subsequent erratic behavior were directly attributed to an 'overindulgence' of alcohol. Efforts to contact Aguto for comment have been unsuccessful
