The New York Inspector General has determined that state regulators failed to do all they could to make sure a poorly maintained limo that killed 20 people in Albany was sidelined.

The operator, Prestige, repeatedly changed the listed number of seats in the 2001 Ford Excursion limo and took other steps to avoid safety regulations, government officials have said.

A judge recently threw out a plea deal that would've allowed Prestige limo driver Nauman Hussain, pictured here at the Schoharie County courthouse on Aug. 31, 2022, in Schoharie, N.Y., to avoid serving any time in prison.
