An inquest heard an electronic monitoring officer failed to report Damien Bendall's comments that he could be capable of murder months before he killed four.
An inquest has heard how a violent criminal predicted murdering his girlfriend and children months before he went on to kill them.
He used a claw hammer to attack Ms Harris, her son John Paul Bennett, 13, daughter Lacey Bennett, 11, and Lacey's friend Connie Gent, also 11, who was staying at the house for a sleepover. Mr McKeown said staff were only qualified and expected to pass on risks to staff in order to keep them safe.
