Tom Hanks' Otto is notoriously grouchy in A Man Called Otto after the passing of his wife Sonya, whose entire life was affected by tragedy.
Sonya's life was struck by tragedy before her offscreen death in A Man Called Otto. In the 2022 movie, Otto struggles to cope with the subject throughout the film and can't find much a purpose for his life after she passes. Once a kind new family moves in across the street, Otto is brought back out of his grumpy shell after much resistance. It is revealed throughout the course of the narrative that the reason Otto has become so pessimistic and sour at the world is because of her death.

Without having much of a family in her absence, Otto resorted to isolating himself and emotionally disconnecting from the world around him. Flashback scenes of a younger Otto explained how he met his wife Sonya and how he once was a bright, friendly person full of optimism.
It was not explicitly stated in A Man Called Otto what type of cancer Sonya died from. All that is known is that Sonya passed away from an unspecified cancer six months before the events of A Man Called Otto begin. Sonya's manner of death is consistent with the character in the original novel that the Tom Hanks movie was inspired by, A Man Called Ove by Fredrik Backman. In the novel, the titular character loses his wife to cancer as well.
Why Sonya's Story Is Even More Tragic Than You Think Flashbacks in A Man Called Otto revealed that Otto and Sonya faced the greatest tragedy of their lives early in their relationship. Shortly after Otto proposed to Sonya she became pregnant. It was supposed to be one of the happiest moments in both of their lives until tragedy struck. After taking a trip to Niagara Falls together, their bus ride home got into an accident and toppled over, killing or injuring many of its passengers.
