A couple of years ago I was having dinner with 10 of my college classmates. As the discussion ebbed and flowed, it turns out that three of them have adolescent grandchildren who are considering or beginning to “transition” to a different gender. While it wasn’t a concept that any of these septuagenarian gentleman understood initially, all three said that they had arrived at a point at which they could say they were supportive of their grandchildren.

While the data generated by this investigation was too limited to support an explanation, the authors offered some possibilities. They wonder if it could result from the normalization of some behaviors that makes it easier for individuals to report their symptoms and complaints. The converse being that having no peers with a mental health diagnosis discourages help seeking. Or it could be that peer social influence is playing a role.

Finally, at the very bottom of our inverted iceberg is the 0.1% of adolescents diagnosed with TGD and are being treated. According to the epidemiologists who performed the Harvard study, these children are being treated in a manner that aligns with the AAP, the World Professional Association of Transgender Health, and the Endocrine Society standards.
