CARE Court, which aims to order treatment for seriously mentally ill people, is among the latest moves in a long history of attempts to balance rights with care

The approach to treating people with mental illness has swung intensely — and controversially — over the decades from locking people up for years in 'insane asylums' to shuttering hospitals and sharply curtailing long-term involuntary treatment. As state facilities emptied out, 'community care' came into fashion, a focus on providing resources in the communities where people live. But the money didn't follow the patients, and the facilities were not enough.

And the law didn't require counties to offer services. San Diego County initially chose not to spend money for community treatment. The county first applied for matching funds in 1962, but put up only $44,250. The attitude in San Diego 60 years ago was, 'Let the state take care of the mentally ill,' Stephen Harmon told the Union-Tribune in 1994, when he was the deputy director of the county's Mental Health Services.
