The Arizona Department of Child Safety (DCS) is under fire again for failing to address persistent problems highlighted in repeated audits. Despite ongoing scrutiny, the agency responsible for protecting vulnerable children continues to struggle with key issues.
The Arizona Department of Child Safety ( DCS ) has been under constant scrutiny over the past decade due to repeated audits revealing persistent problems. A 2023 audit highlighted that the agency, responsible for protecting Arizona 's most vulnerable children, has not fully implemented 42 out of 58 recommendations made from six special audits conducted between 2016 and 2021.
These unaddressed issues encompass critical areas like finding missing children and submitting timely reports to courts, which have resulted in postponements of hearings. Since becoming a standalone agency in 2014, DCS has faced numerous audits, yet a comprehensive evaluation of its most crucial function — investigating child abuse and neglect — has never been conducted, as acknowledged by Arizona Auditor General Lindsey Perry in 2024. This oversight is currently being rectified with an ongoing audit examining DCS's response to reports of abuse or neglect. The agency's repeated failures have sparked considerable frustration among lawmakers. During a 2024 Sunset Review, many expressed a lack of trust in DCS and opted for a four-year review rather than the standard 10 years. Lawmakers criticized the agency's inability to address ongoing issues and called for greater accountability. Former Director David Lujan acknowledged the need for improvement but maintained that the agency was working on implementing changes. However, Lujan subsequently resigned in 2025 to pursue another position. The current director, Kathryn Ptak, maintains that the agency disagrees with some audit findings, claiming ambiguity in certain recommendations makes compliance difficult. She asserts that change takes time and emphasizes the agency's collaboration with the Auditor General's Office to implement recommendations swiftly
