EpiScalp, a new AI-powered tool, uses algorithms trained on dynamic network models to map brainwave patterns and identify hidden signs of epilepsy from a single routine EEG, potentially reducing misdiagnoses and improving patient care.
A new artificial intelligence (AI) tool called EpiScalp has the potential to revolutionize epilepsy diagnosis by accurately identifying hidden signs of the condition from a single routine electroencephalogram ( EEG ). Developed by researchers at Johns Hopkins University, EpiScalp utilizes algorithms trained on dynamic network models to map brainwave patterns and differentiate between epilepsy and nonepilepsy, even in cases where initial EEG s appear normal.
This breakthrough could significantly reduce misdiagnoses, which currently affect up to 30% of epilepsy patients. \Epilepsy affects approximately 60 million individuals worldwide, and an estimated 8%-10% of the general population will experience a seizure during their lifetime. However, only 2%-3% of these individuals will develop epilepsy. Scalp EEG is the primary method for diagnosing epilepsy and assessing the risk of future seizures. Traditionally, EEG interpretation relies on visual analysis of the recording, focusing on interictal epileptiform discharges (IEDs) and slowing of waveform activity. However, due to the sporadic nature of IEDs, the sensitivity of scalp EEG in detecting abnormalities ranges from 29% to 55%, leading to a high misdiagnosis rate. Moreover, EEG readings are subjective and can vary depending on the reader. \To address these limitations, the EpiScalp team analyzed routine scalp EEGs from 218 patients who had suspected epilepsy but initially normal EEGs. These patients were subsequently diagnosed based on admission to an epilepsy monitoring unit (EMU). Using this dataset, they trained a logistic regression model called EpiScalp that incorporates spectral and network-derived EEG features to distinguish between epilepsy and nonepilepsy. EpiScalp demonstrated high accuracy in differentiating between the two conditions, even in cases where initial EEGs appeared normal. The tool achieved an area under the curve of 0.940, an accuracy of 0.904, a sensitivity of 0.835, and a specificity of 0.963. In a test group of 20 patients, EpiScalp correctly predicted the diagnosis in 80% of cases. This represents a significant advancement in epilepsy diagnosis, as it provides an objective measure of epilepsy likelihood from previously uninformative EEGs. \The researchers are currently conducting a larger prospective study to further validate their findings across three epilepsy centers and have filed a patent for the EpiScalp technology. This promising tool has the potential to improve the accuracy and efficiency of epilepsy diagnosis, leading to earlier interventions and better patient outcomes
