The first sham-controlled trial of atrial fibrillation ablation with pulmonary vein isolation shows a placebo-resistant benefit, but questions remain about how it works, writes John Mandrola, MD.

It's finally here. Twenty years and millions of procedures later, Rajdip Dulai, MBBS, and colleagues in the United Kingdom have presented positive results ofMany of my colleagues have long denied the need for a placebo-controlled trial. They argue that trials of ablation vs drugs show that ablation is superior in reducing AF episodes.

The authors measured QOL with three different scores. All three improved significantly more in the ablation arm. I had AF that resolved without a procedure over a year. At that time, my main worry was not AF but a deadly complication of ablation, such as atrial-esophageal fistula.
